Frequently Asked Questions about Patient Records

Thank you for trusting Transcend Veterinary with your patient's urgent care. We value collaboration and want to ensure you have everything needed to continue their care. Below are answers to common questions about accessing records, bloodwork, and imaging. 

We email patient records to the primary (referring) veterinarian within 24 hours of the visit — typically the same day the patient is seen.


Each email clearly states:

  • The patient's name
  • The client's name
  • The date of visit
  • The reason for the visit
     

At the bottom of the email, you’ll find a link that will take you a separate website where you can download:

  • A full SOAP summary with exam findings, diagnostics, assessment, and treatment 
  • The discharge instructions provided to the client - included in SOAP
  • Any imaging performed, such as radiographs or ultrasound - included in SOAP
  • Any bloodwork results (CBC, chemistry, etc.) 


Follow the download link included at the bottom of the email you received.


This link will take you to a secure webpage that contains all visit documents. Look for the section labeled “Laboratory.” Under this section, you’ll find PDF documents of any bloodwork that was performed, such as CBCs or chemistry panels.


If you have trouble locating the link or accessing the page, please contact us and we’ll be happy to assist.


Just like with the bloodwork, follow the download link included at the bottom of the email you received.


This will take you to a secure webpage that contains all visit documents. Look for the section labeled “Medical Record.” Under this section, download the PDF which includes the SOAP, Discharge Instructions, and any imaging performed.


To access the images:

  • Open the PDF from the Medical Record section.
  • Under the SOAP, scroll to the heading titled “Imaging Notes.”
  • You’ll see a blue text link — this link will take you directly to the radiographs or ultrasound images.
  • From there, you can view and download the images for import into your medical record software.


If the link expires or doesn’t work, just let us know and we’ll send a new one right away.
